COMPANY POLICIES FOR ONLINE SERVICES
1. Required Forms
In addition to this form, Client will be required to sign and return the following forms to Brunner Sports Development & Fitness, LLC prior to receiving a fitness consultation, training program design, online coaching services, or beginning any personal training program:
A. Waiver, Release, and Assumption of Risk (Online Services)
B. Physical Activity Readiness Questionnaire (PAR-Q)
C. Physician’s Referral (if required)
D. Health History Questionnaire
E. Training Service Agreement
2. Medical Clearance Requirements
If Client has any of the following physical conditions, Client may be required to obtain Medical Clearance and Physician Consent prior to participation:
A. Hypertension (>145/95 mm Hg)
B. Hyperlipidemia (cholesterol >220 mg/dl or total cholesterol-to-HDL ratio >5.0)
C. Diabetes
D. Family history of heart disease prior to age 60
E. Smoking
F. Abnormal resting EKG
G. Any other condition that Brunner Sports Development & Fitness, LLC, in its sole discretion, deems to present an unreasonable risk to Client’s health during participation in fitness evaluations or exercise programs.
3. Session Duration
Unless otherwise arranged, personal training sessions and program design consultations (collectively referred to herein as “Sessions”) will last up to ___ hours and ___ minutes.
To provide the best service to all Clients, Brunner Sports Development & Fitness, LLC cannot guarantee extensions beyond the scheduled Session time. When scheduling permits, Clients may request additional time at the current hourly Session rate.
4. Rates and Pricing
Rates for services provided by Brunner Sports Development & Fitness, LLC are subject to change at any time. However, prepaid services that remain unused at the time of a rate adjustment will be honored at the previously paid rate.
5. Scheduling and Payment
Appointments are available on a first-come, first-served basis by appointment only.
Sessions, whether purchased individually or as part of a package or membership, must be paid for at the time of booking. Clients may schedule prepaid Sessions in advance.
6. Tardiness and No-Show Policy
Clients are expected to be prepared to begin their Session at the scheduled time.
Time lost due to Client tardiness cannot be extended beyond the scheduled Session end time, as doing so may impact subsequent appointments.
Unless prior arrangements have been made, a Client who is more than fifteen (15) minutes late will be considered a “no-show.” No refunds or credits will be issued for no-show appointments.
7. Cancellation Policy (In-Person Sessions)
A. Clients are permitted approximately one cancellation per month that may be rescheduled.
B. Qualifying cancellations may result in a credit that can be applied toward a future Session or another service offered by Brunner Sports Development & Fitness, LLC.
C. Cancellations must be made at least twenty-four (24) hours in advance in order to qualify for a credit. Due to scheduling limitations, cancellations made with less than 24 hours’ notice will be charged in full and will not qualify for a credit.
D. Cancellations must be submitted by calling or texting 305-432-0814 or by emailing DustenBrunner@BRunFit.com.
E. Credits issued for missed Sessions must be used within the same calendar month in which the missed Session occurred or they will expire.
F. Session credits do not roll over from month to month.
G. Clients who travel frequently for work must provide at least one month’s notice with exact dates in order to qualify for reduced monthly billing adjustments. Failure to schedule before the next billing cycle may result in forfeiture of missed Sessions.
H. If Brunner Sports Development & Fitness, LLC must cancel a scheduled Session, Client will receive either a rescheduled Session or a credit applied toward the next monthly payment.
I. If a Client elects not to continue services into the following month, any remaining credits may be applied toward continued access to online coaching services, including Precision Nutrition ProCoach access, until credits are exhausted.
8. Accepted Payment Methods
Payment is due at the time a Session is booked.
Brunner Sports Development & Fitness, LLC accepts cash, checks, credit cards, and approved electronic payment methods. Payments may also be made online through:

9. Facility Rules
Clients agree to observe and comply with all rules, policies, and regulations of any gym, facility, field, or training location where workouts or Sessions take place.
10. Attire and Hydration
Proper athletic attire, including shirts and athletic shoes, is required at all times during Sessions.
Clients are encouraged to bring water and maintain adequate hydration throughout exercise participation.
11. Client Exercise Rights
Clients have the right to stop any exercise or workout at any time.
If an exercise becomes painful, uncomfortable, or inappropriate due to injury, limitation, or medical concern, Brunner Sports Development & Fitness, LLC may modify, substitute, or remove the exercise as deemed appropriate.
Client participation is voluntary, and Clients are encouraged to communicate openly regarding pain, discomfort, fatigue, or concerns during exercise.
12. Results Disclaimer
Results vary between individuals and are influenced by multiple factors including exercise consistency, nutrition, sleep, recovery, stress management, and lifestyle habits.
Brunner Sports Development & Fitness, LLC makes no guarantees regarding specific fitness, performance, health, or weight-loss outcomes.
Client acknowledges responsibility for their own participation, adherence, and lifestyle decisions.
13. Privacy and Confidentiality
Brunner Sports Development & Fitness, LLC respects Client privacy and confidentiality.
To provide services effectively, certain personal and health-related information may be collected from Clients. All information collected will be treated as strictly confidential and will not be shared, sold, or redistributed to third parties except:
as necessary to provide contracted services,
with Client authorization,
or as otherwise required by law.
Information collected is maintained solely for business records, coaching purposes, and service delivery.
14. Policy Changes
All Terms, Conditions, and Policies are subject to change at any time without prior notice.
